Our staff will file your primary and secondary insurance claim for you. Patients will be responsible for the payment of deductibles, co-pays, and co-insurance, as well as any non-covered items at the time of service. Should insurance reimburse these items, any credit will be refunded to you.
Most HMO plans require you to obtain a referral from your primary care physician prior to visiting our practice. Please check with your insurance, and obtain this pre-authorization, before your appointment.
