If this is your first visit to our office, please bring your insurance card and driver's license with you. We ask that you arrive fifteen minutes prior to your appointment to complete the necessary patient forms, or print them now, fill them out in advance, and bring them with you.

Our staff will file your primary and secondary insurance claim for you. Patients will be responsible for the payment of deductibles, co-pays, and co-insurance, as well as any non-covered items at the time of service. Should insurance reimburse these items, any credit will be refunded to you.
HMO Insurance
Most HMO plans require you to obtain a referral from your primary care physician prior to visiting our practice. Please check with your insurance, and obtain this pre-authorization, before your appointment. If a referral is not obtained prior to your appointment, you may be responsible for the full amount of the visit.
Surgical Procedures
If your insurance carrier requires pre-certification of your planned surgical procedure, our staff will assist you in obtaining this approval prior to your procedure.
